Supports patients in a behavioral health and addiction treatment setting by monitoring high-risk individuals, facilitating recovery groups, documenting activities and incidents, promoting engagement, maintaining program guidelines, and collaborating with nurses, counselors, medication specialists, and leadership.

Supports patients in a behavioral health and substance use treatment setting by monitoring high-risk individuals, facilitating recovery groups, documenting observations, providing redirection and individual support, and collaborating with nurses, counselors, medication specialists, and program leadership. The role also tracks program evaluations, maintains patient records, supports facility guidelines, and promotes recovery-oriented socialization and wellness.

Provides behavioral health and justice-involved patient navigation, including interviews, assessments, care planning, case management, psychoeducation, referrals, recovery-support development, and coordination with correctional, probation, parole, and community agencies. Maintains patient records, tracks caseload tasks, supports transportation and outreach, participates in multidisciplinary teams, and assists patients with accessing treatment and community resources.
