Leads supported community living services by supervising direct support professionals, managing participant caseloads, developing treatment and safety plans, coordinating schedules, monitoring outcomes and budgets, maintaining case documentation, providing transportation, and building relationships with referral agencies and funders.

Provides supported employment services for participants with barriers to independence. Manages caseloads, delivers or coordinates job coaching, develops service plans, maintains case documentation, gathers employer feedback, supports workplace accommodations, coordinates community resources, and monitors program expenses. The role requires employer and funder relationship management, accurate reporting, transportation assistance, and travel between job sites.

Provides supportive community living services to program participants, including relationship-building, resource coordination, treatment-plan monitoring, service facilitation, expense and billable-hour management, case documentation, progress reporting, and transportation. Assists with individualized service plan development and helps participants access rehabilitation and community resources.
