Provides confidential assessment, problem-solving, information, and referral assistance to clients with complex health and social needs. Handles phone, chat, and text contacts; screens for community programs; documents interactions; identifies solutions to access barriers; conducts follow-up and satisfaction surveys; and maintains accurate records.
