Conduct in-home and video Level of Care assessments for individuals on the Developmental Disabilities Waiver. Manage assigned caseloads, maintain case records and documentation, bill monthly, coordinate referrals to support services, ensure HIPAA and CARF compliance, build community relationships, and travel statewide to perform assessments.

Manage fiscal compliance, reconciliation, billing, and financial analysis for assigned grants and programs. Prepare budgets, spending plans, monthly billings, audit documentation, and variance reports. Ensure grant-specific compliance, accurate timekeeping coding, HIPAA and CARF standards, and implement process improvements. Provide ad hoc reporting and support to leadership; travel in-state and out-of-state as needed.

Provide case management and supportive services to veterans, including outreach, intake, referrals, budgeting, billing, documentation, and community coordination. Maintain caseload, ensure program compliance (HIPAA, CARF), submit required reports and billing, and support participants with housing stability and resource navigation. Travel locally as needed and participate in team meetings and professional development.
