Goddard Riverside Community Center Benefits Overview

Provides case management and social service support to senior residents. Responsibilities include benefits navigation, resource referrals, assessments, documentation, advocacy, intake support, group activities, basic technology assistance, home visits, and coordination with internal and external service providers to promote resident independence, health, safety, and quality of life.

Provide individualized case management for up to 30 participants ages 17–24. Complete biopsychosocial assessments, treatment plans, documentation, referrals, benefits screening, workshops, and progress tracking. Help participants overcome barriers related to education, employment, economic stability, housing, healthcare, mental health, and other social services. Build community partnerships, coordinate resources, navigate complex systems, and collaborate with multidisciplinary staff.

Supports daily administrative and office operations for a homeless outreach program. Responsibilities include managing calls and visitors, scheduling, invoices, supplies, equipment, mail, inventory, client charts, data collection, database updates, and reports. The role requires strong organization, multitasking, communication, and a calm, professional approach when working with clients and community partners.
