Assists the Social Service Director with planning, implementing, evaluating, and coordinating social service programs in a skilled nursing facility. Supports identification of residents’ emotional and social needs, collaborates with administration and care staff, helps maintain department policies, and participates in community planning. The role requires compassionate resident support, communication with families and personnel, independent judgment, teamwork, and willingness to work a Friday-through-Tuesday schedule.

Provide compassionate nursing care to residents in a 76-bed skilled nursing facility, supporting short-term rehabilitation, long-term care, and subacute services. Responsibilities include delivering care as a Licensed Vocational Nurse and communicating understandably in English. New graduates are welcome. Employment requires successful completion of a background check, pre-employment physical, and drug screening.

Provides front-desk clerical support at a skilled nursing and rehabilitation facility. Answers and directs telephone calls, pages personnel, responds to internal requests, releases information according to policy, and maintains resident, emergency contact, department extension, and call-restriction lists. Requires tactful interaction with residents, families, visitors, staff, and the public, along with computer, typing, word-processing, and 10-key skills.
