Provides case management for individuals experiencing homelessness by conducting assessments, developing individualized care plans, maintaining HMIS records, coordinating referrals and services, advocating with community partners, arranging appointments and transportation, facilitating programs, protecting confidentiality, and responding to behavioral concerns. The role also requires participation in training, including de-escalation, motivational interviewing, CPR, and First Aid.

The role also involves crisis support, transportation coordination, appointment scheduling, behavioral de-escalation, and participation in training. Requires a relevant combination of education and experience, documentation skills, and successful completion of a Live Scan background check.
