Perform in-home HDM and EISEP assessments; complete NYSOFA/OAA documentation, financial and nutrition evaluations; coordinate benefits and care plans; maintain client files and follow-up contacts; refer to dietitian and community resources; oversee outreach, staff training, and service quality; manage service changes, terminations, and eligibility. Ensure client safety and regulatory compliance.

Facilitate psychiatric rehabilitation groups and provide individual services, relapse prevention, and crisis intervention. Collaborate on individualized recovery plans, maintain electronic case records per OMH/Medicaid, coordinate discharge planning, and make referrals and linkages to supports and treatment.

Supervise, train, and evaluate janitorial staff to meet contract specifications and agency quality standards. Perform and oversee cleaning tasks, manage equipment and supplies, maintain vehicle records, transport staff to off-site locations, and participate in training. Ensure work is completed on schedule and represent the agency professionally.
