Completes MDS assessments and discharge tracking for nursing facility residents in accordance with OBRA regulations. Identifies clinical triggers, develops and updates interdisciplinary care plans, coordinates assessment reference dates, schedules and leads care conferences, and follows up on resident care needs. Participates in interdisciplinary team activities, provides direct patient care as needed, supports nursing staff, attends in-service meetings, and promotes a positive work environment.

Provide direct nursing care as the LVN Charge Nurse in a skilled nursing facility: assign and supervise staff, maintain resident care plans and records, administer medications and treatments, transcribe orders, oversee unit operations, ensure regulatory compliance, and communicate with physicians, families, and administration.
