Provides hands-on care and support to geriatric residents in a skilled nursing facility. Responsibilities include assisting with daily living activities, recording vital signs, responding to assistance calls, documenting care, transporting residents, monitoring condition changes, providing companionship, maintaining safety and cleanliness, and encouraging resident engagement.

Provides hands-on support to nursing staff and geriatric residents, including assistance with daily living activities, vital signs, transportation, documentation, companionship, safety, and maintaining a clean environment. Observes and reports resident condition changes while promoting comfort, engagement, and well-being.

Provides clinical nutrition care to nursing home residents through nutrition assessments, medical nutrition therapy, individualized care plans, enteral nutrition recommendations, education, and ongoing monitoring. Collaborates with interdisciplinary teams, completes MDS assessments, applies evidence-based nutrition practices, supports resident-centered care, and conducts quality assurance audits.
