Teams at FrontLine Service

Provides supportive services and case management to adults transitioning from institutional care to community living and homelessness. Supports individuals with substance use diagnoses, with or without severe mental illness, and maintains progress notes and regulatory documentation. Works with the Forensic Treatment team under general supervision during weekday first-shift hours.

Provide community-based mental health support by advocating for clients, connecting them to housing, healthcare, and substance-use services, and conducting outreach and transportation to ensure access to care. Receive clinical supervision and professional development opportunities.
