Supports community outreach, health fairs, stakeholder partnerships, and patient engagement initiatives. Provides information about healthcare services, assists with enrollment and community-resource referrals, and promotes access for underserved Medicaid and Medicare populations. Represents the organization at events, collaborates with marketing, creates outreach content, and facilitates community relationships. Requires bilingual English-Spanish communication, evening and weekend availability, travel, and the ability to work across diverse communities.

Provide primary care to patients at the clinic, delivering exams, diagnoses, treatment, referrals, continuity of care, and clinical oversight. Participate in community health planning, quality improvement, protocol development, team leadership, education, and required administrative duties. Maintain medical records, review labs and imaging, and meet encounter and weekly hour targets.

This role involves diagnosing and treating mental health disorders, providing psychoeducational services, and ensuring continuity of patient care across various clinics.
