Provides licensed nursing support through inbound and outbound patient calls, telephone symptom assessment, care coordination, appointment scheduling, referrals, medication refills, prior authorizations, documentation, patient education, and escalation of urgent issues. Maintains accurate EHR records, protects patient confidentiality, verifies insurance, supports quality improvement, and routes requests to appropriate care team members while delivering professional customer service.

Provide comprehensive eye and visual system exams, diagnose and manage eye diseases, prescribe corrective lenses and therapeutic drugs as allowed, provide pre/postoperative care for ocular surgery, refer to ophthalmology when needed, and promote preventive eye health and patient satisfaction.

Provides individual and group nutritional counseling and medical nutrition therapy; develops care plans and education materials; consults with clinicians and physicians on nutrition-related treatment; evaluates, documents, and monitors patients' nutritional status; participates in interdisciplinary teams and maintains RD licensure and required certifications.

Provide comprehensive psychiatric care including diagnosis, treatment, hospitalization coordination, and preventive care. Participate in PCMH activities, supervise behavioral health and mid-level staff, be available for emergencies/on-call, engage in quality assurance and journal club activities, and comply with HIPAA, OSHA, and CLIA policies.
