The Referral Coordinator connects Forge Health's services to the community by building partnerships, executing outreach initiatives, and increasing referrals through relationship management and community engagement.

Serve as the clinic's first point of contact, combining front desk operations, peer support, and administrative tasks. Duties include greeting and orienting clients, scheduling and phone management, facility and supply oversight, EMR documentation (including UDS uploads), processing medical record requests, outreach to engage clients, and co‑facilitating peer groups while collaborating with clinical and billing teams.

Build and maintain referral relationships across Northern NJ to increase referrals, execute outreach strategies, represent Forge at community events, host educational forums, manage pipelines in CRM, collaborate cross-functionally, and track outreach performance.
