Screens employees, visitors, and essential personnel for potential COVID-19 exposure; maintains confidential screening records; supervises resident visits; communicates professionally with residents and families; and assists with clerical duties. Requires regular attendance, weekend/evening/holiday availability, clear communication, and the ability to lift up to 50 pounds.

Coordinates the RAI and MDS processes in a skilled nursing facility, ensuring timely, accurate documentation, care plans, regulatory compliance, reimbursement support, and government submissions. Reviews assessments, tracks Medicaid and Medicare requirements, supports audits and surveys, trains staff, monitors documentation, coordinates interdisciplinary care planning, and maintains clinical records. Requires a current LVN license and knowledge of nursing facility reimbursement, OBRA regulations, quality indicators, and care planning.

Assists long-term care residents with activities of daily living, mobility, grooming, comfort needs, call lights, housekeeping-related tasks, laundry, transportation to activities, and safety procedures. Observes residents and reports changes to nursing staff. Requires respectful resident care, communication skills, physical ability to lift up to 50 pounds, regular attendance, and availability for assigned weekends, evenings, and holidays.
