Teams at Focused Post Acute Care Partners

Provides direct nursing care to long-term care residents, implements physician orders, assesses patient conditions, develops care plans, documents in PointClickCare, and supervises licensed and nonprofessional staff. Ensures resident safety, infection control, regulatory compliance, effective communication, and timely response to urgent situations.

Assists with meal preparation, delivery, dishwashing, meal-service cleanup, and scheduled kitchen sanitation. Maintains clean and safe work areas, follows food safety, infection-control, PPE, and chemical-handling procedures, operates equipment safely, reports repairs, supports positive resident and guest service, and performs other assigned duties. Requires regular attendance and the ability to lift up to 50 pounds while working assigned weekends, evenings, and holidays.
