Teams at Family & Children's Center

Provides one-on-one, community-based support to children, adults, and families by teaching daily living, money management, time management, social, conflict-resolution, and self-advocacy skills. Collaborates with families and care teams, follows individualized service plans, documents services, maintains billing records, and builds trusting client relationships. The role requires local travel to homes and community settings, crisis-intervention knowledge, accurate recordkeeping, and dependable independent work.

Supervise parent-child visits, transport children, monitor client compliance through in-person and phone checks, conduct breathalyzer and urine tests, report suspected abuse or neglect, maintain safety, and complete service documentation. The role requires reliable attendance, mandated reporting, family-centered communication, and flexibility for evening, weekend, and holiday shifts.

Provides home-based support to families with children ages 0–5 through the Healthy Families America model. Duties include delivering direct services, assessing family histories and strengths, planning weekly visits, teaching self-sufficiency and stress-management skills, documenting activities in the EHR, attending staff meetings, reviewing cases with a clinical supervisor, and completing annual professional development.
