Provides direct nursing care to PACE participants across care settings. Conducts assessments, implements provider orders, delivers individualized services, monitors interventions, contributes to care plans, delegates appropriately, responds to emergencies, documents care, and collaborates with interdisciplinary teams. The role also includes participant education, staff training, on-call and weekend rotations, and supporting frail or elderly adults.

Provides direct nursing care to PACE participants across care settings. Responsibilities include implementing provider orders, conducting physical and psychosocial assessments, delivering individualized care, monitoring interventions, contributing to care plans, responding to emergencies, documenting care, coordinating with interdisciplinary teams, delegating appropriately, training staff, and participating in on-call rotations. Requires Massachusetts RN or LPN licensure, CPR and Alzheimer’s certification, reliable transportation, and recent experience caring for adults, including frail or elderly populations.

Develops and implements individualized care plans, performs assessments, administers medications and treatments, coordinates interdisciplinary care, delegates tasks, monitors interventions, responds to emergencies, documents care, and advocates for participants. The role also includes caregiver communication, staff training, committee participation, on-call rotation, and clinical testing such as EKGs, bladder scans, point-of-care testing, and glucometer testing.
