Provide direct patient care under RN supervision in a home-health setting: implement plans of care, perform treatments, document clinical notes, educate patients/caregivers, coordinate services with other disciplines, and report concerns to RN/manager.

Ensure adherence to the organization's Code of Conduct and Compliance Policies by acknowledging obligations, certifying ongoing compliance during employment, and understanding that violations may result in disciplinary action up to termination.

Act as liaison between agency and community to develop referral relationships, promote home health and hospice services, meet admit quotas, maintain CRM pipeline, execute outreach and in-services, manage territory coverage and sales administration while ensuring compliance and fiscal stewardship.
