Provide resident-centered skilled nursing in a Green House® senior living setting. Administer medications and treatments, assess and monitor residents, update individualized care plans, and collaborate with caregivers, therapists, providers, and families to support dignity and quality of life.

Provide resident-centered nursing care including medication administration, assessments, individualized care plan development, staff supervision, interdisciplinary coordination, documentation in EMR, infection control, and quality/compliance activities to support long-term care residents' health and well-being.

Provide direct nursing care to long-term care residents: administer medications and treatments, assess and update individualized care plans, supervise caregiving staff, coordinate with physicians and therapists, document conditions and care in EMR, participate in infection control and quality initiatives, and advocate for resident dignity and family education.

Admit and assess new patients to the Transitional Care Unit, document orders in the EMR, initiate individualized care plans, coordinate with interdisciplinary teams, educate patients/families, monitor condition changes, and support discharge planning while providing compassionate, customer-focused care.
