Provides direct, onsite case management for residents experiencing homelessness, behavioral health conditions, substance use challenges, and housing instability. Responsibilities include intake assessments, individualized service plans, referrals, benefits enrollment, crisis response, housing retention advocacy, wellbeing checks, group facilitation, documentation, discharge planning, and community activities. Requires collaboration with residents, property management, service providers, and multidisciplinary teams.

Provides intensive case management to residents experiencing homelessness, behavioral health conditions, substance use challenges, and other barriers. Conducts assessments, develops service plans, coordinates referrals and benefits, supports housing stability, performs wellbeing checks, facilitates therapeutic groups, documents services, assists with discharge planning, and responds to crises. The role also involves conflict resolution, community activities, collaboration with property management and external agencies, and direct onsite participant engagement.

Provides direct, onsite support to shelter participants by managing intake, orientations, wellness checks, documentation, rule compliance, referrals to case management, and participant requests. Maintains a safe, clean, and orderly shelter environment; monitors participant wellbeing, handles property, responds to concerns, and follows confidentiality, safety, crisis-resolution, and agency procedures.
