Provides individualized job coaching, community-based support, independent living and mobility training for individuals with disabilities. Develops training plans, teaches workplace skills, recommends accommodations, advocates for inclusion, collaborates with employers and care teams, and documents participant progress. Requires transportation between community and job sites, accurate reporting, and support in varied environments.

Implements individualized program and employment plans for participants with disabilities through intake, assessment, training, job coaching, placement, advocacy, documentation, and progress monitoring. Builds employer relationships, develops job opportunities, supports workplace accommodations, and coordinates with families, agencies, and service providers. Maintains participant and employer records, submits reports, ensures compliance, and provides ongoing employment support.

Provide individualized job coaching, employment and independent-living training, transportation and mobility support, advocacy, and workplace accommodations for adults with disabilities or other barriers. Develop and adjust training plans, document participant progress, collaborate with employers and case managers, and promote independence and community inclusion across office, community, and job-site settings.
