Teams at Ennoble Care

Serve as primary hospital liaison to identify patients eligible for hospice/palliative services, educate patients/families, perform nursing assessments, develop and initiate goal-aligned care plans, coordinate discharges, and build relationships with facility staff and referral sources.

Provide on-call hospice nursing care in patient homes across Loudoun/Arlington/Fairfax VA: triage calls, perform visits, administer medications, relieve end-of-life symptoms, coordinate with physicians and caregivers, maintain records, assist with paperwork, and provide emotional and post-mortem support to families.

Provide hospice LPN care in patients' homes: assist RNs with care plans and specialized procedures under physician orders, administer medications, support interdisciplinary group (IDG) collaboration, reinforce family teaching, offer emotional support, and promote patient dignity and comfort during end-of-life care.
