Coordinate and manage patient scheduling for home-based primary, palliative, and hospice care. Prioritize urgent appointments, handle voicemails, update patient and insurance information, perform insurance preauthorizations, and maintain provider appointment quotas. Communicate with patients, providers, and referring sources to ensure timely scheduling and accurate chart notations. Manage high call volumes and use multiple software databases to support smooth operations.

Provide medical oversight for hospice and palliative care patients, perform annual wellness exams, certify and re-certify hospice eligibility, collaborate with interdisciplinary teams, manage symptom control and medications, attend IDG meetings, develop protocols, educate staff and community, and ensure regulatory compliance.
