Teams at Eminence Home Care

Develop and expand referral partnerships across hospitals, rehabilitation facilities, physician practices, senior communities, and organizations. Manage a defined territory, conduct field visits, generate referrals and new accounts, nurture relationships, coordinate with intake to convert referrals into care starts, track activity in a CRM, analyze referral trends, and report weekly results. The role is approximately 80% field-based and requires organized planning, follow-up, networking, and community representation.

Develops referral partnerships and expands community presence for a non-medical home care agency. Responsibilities include prospecting hospitals, rehabilitation facilities, senior communities, physician offices, and community organizations; conducting field visits and presentations; managing territory outreach; coordinating referral follow-up; tracking activity and performance in a CRM; and collaborating with intake and operations teams to convert referrals into new clients.

Manage personnel documentation, onboarding, payroll and benefits support for home care staff. Maintain licenses and compliance records, coordinate bilingual orientation, provide system support for HHAeXchange and Arla, respond to payroll/benefits inquiries, and collaborate with Compliance to ensure trainings and certifications are completed.
