Conduct outreach and engagement with people experiencing homelessness, provide essential supplies and service referrals, encourage shelter and resource use, maintain knowledge of community resources, and coordinate integrated support with other providers. The role requires lived experience of homelessness, mental health, or substance use recovery, strong relationship-building skills, community-based teamwork, and reliable transportation.

Provides intensive, person-centered case management to adults with severe and persistent mental illness in community or residential settings. Responsibilities include outreach, side-by-side interventions, skills training, treatment planning, community integration, recovery support, and helping clients pursue employment and leisure goals. Collaborates with multidisciplinary teams and participates in rotating evening and weekend on-call coverage.

Provides administrative and clinical leadership across stabilization, tenancy preservation, and enhanced navigation programs. Oversees service expansion, quality assurance, staff recruitment, partnerships, procurement, and program reporting. Offers clinical guidance for high-risk client needs using trauma-informed care, harm reduction, and Housing First principles. Facilitates multidisciplinary meetings and collaborates with senior leadership to improve systems of care and operational effectiveness across multiple Massachusetts locations.
