Develops and manages relationships with healthcare referral sources through in-person sales calls, needs assessment, service promotion, customer support, account reporting, and follow-up. Coordinates with account managers, clinical teams, and internal partners to support sales plans, retain customers, address patient concerns, and promote services. Reports to the Area Vice President of Sales.

Generates admissions and grows patient census within an assigned territory by selling and marketing home care programs to healthcare professionals, patients, and families. Builds referral relationships, identifies decision-makers, presents solutions, meets business objectives, and collaborates with internal teams. Requires independent initiative, strong sales performance, problem-solving skills, healthcare professional communication, a dependable vehicle, valid driver’s license, and current auto insurance.

Provides home health admissions and start-of-care visits for new patients. Completes comprehensive clinical, functional, psychosocial, and environmental assessments; develops individualized care plans; documents in the EMR; educates patients and caregivers; identifies risks; coordinates interventions; communicates changes to physicians and clinical leaders; and ensures regulatory, quality, and HIPAA compliance across the assigned Massachusetts coverage area.
