Provides personal assistance and overnight care to adults with intellectual and developmental disabilities. Responsibilities include medication support, health and safety monitoring, assistance with daily living activities, meal preparation, hygiene, household tasks, community outings, skill development, goal achievement, and maintaining compliant progress documentation.

Provides individualized after-school and occasional weekend support to individuals needing community and daily-living assistance. Responsibilities include supervision, promoting independence and social engagement, supporting activities, maintaining a safe environment, communicating with families and care teams, following support plans, and completing documentation. Requires a high school diploma or GED, age 18 or older, a valid driver’s license, active auto insurance, background and drug screenings, and strong communication skills. One year of related experience is preferred but not required.

Lead launch and oversee iRISE supervised residential and in-home supports for adults with SMI. Manage daily operations, ensure person-centered plans, enforce regulatory compliance (907 KAR 16:015, HIPAA), conduct QA of EHR/EVV, oversee incident reporting, recruit and supervise program staff, and coordinate with external case managers and CHFS.
