Teams at David Raines Community Health Centers

Plans and coordinates community outreach, partnership development, health fairs, mobile medical unit activities, fundraising events, volunteer engagement, and advocacy initiatives. Represents the organization at public events, develops stakeholder relationships, supports business development and funding efforts, manages event logistics and outreach databases, tracks engagement metrics, and assists with marketing and communications initiatives. Requires collaboration with internal teams and community partners, including occasional extended and weekend work.

Provides comprehensive pediatric medical care, including examinations, diagnosis, treatment, referrals, hospital and emergency care, follow-up, patient education, health maintenance, documentation, and chart completion. Participates in quality improvement, health services planning, committee work, staff coordination, and productivity initiatives. Requires rotating after-hours and weekend coverage, EHR documentation, and compliance with clinical standards.

Provide case management support to patients with chronic or acute illnesses, including education, referrals, appointment coordination, and follow-up. Handle patient calls, maintain confidential health and financial information, complete administrative documents, manage calendars and supplies, and ensure accurate, secure documentation in compliance with privacy regulations and organizational policies. The role may rotate among multiple community health center locations.
