Serves as a hospital-based liaison for patients requiring durable medical equipment. Coordinates discharge equipment needs, benefits verification, inventory, hospital deliveries, home equipment arrangements, patient and caregiver education, paperwork, and consignment accounts. Maintains compliance with HIPAA and applicable regulations while supporting branch operations and developing hospital partnerships. The role requires customer service, empathy, communication, occasional travel, and the ability to lift equipment.

Coordinates hospital discharge support for patients requiring durable medical equipment. Attends discharge planning meetings, records patient needs, verifies benefits and inventory, delivers and explains equipment, completes paperwork, manages consignment inventory, supports branch operations, and maintains compliance with HIPAA and applicable healthcare regulations. Builds relationships with hospital partners and assists patients, families, and caregivers during the transition home.
