Support social services in a long-term care and short-term rehab nursing center. Assist Social Services Manager with resident admissions, psychosocial assessments, care and discharge planning, documentation, and ensuring positive patient experiences while coordinating treatment plans.

Manage daily operations of a nursing unit, ensuring high-quality resident care and regulatory compliance. Supervise, mentor, and support nursing staff; coordinate interdisciplinary care with physicians and therapy; monitor resident conditions and documentation; address resident and family concerns; and promote a positive, efficient work environment.
