Generates referrals and admissions growth for a home health, palliative, hospice, and wellness agency. Builds relationships with physicians, hospitals, facilities, case managers, patients, families, and community organizations; conducts sales outreach, presentations, and networking; educates stakeholders on agency services; coordinates with intake and clinical teams; tracks referral and admission performance; and develops strategies to expand the referral network and achieve business development targets.

Supports external communications by coordinating schedules and deadlines, editing and proofreading materials, producing photo, video, and audio content, managing social media publishing, organizing communications calendars, developing collateral, tracking opportunities, and coordinating with external vendors.

The Major Gift Officer manages a portfolio of $25,000+ donors and prospects, identifying, cultivating, soliciting, and stewarding major gifts. Responsibilities include developing prospect pipelines, securing five- and six-figure donations, building relationships with donors and community leaders, coordinating donor engagement opportunities, supporting strategic solicitations, conducting prospect research, tracking fundraising data, and maintaining accurate database records. The role partners with organizational leadership, clinical and administrative teams, volunteers, and the Charitable Fund Board.
