Teams at Concordance Healthcare Solutions

Provide career coaching, classroom instruction, one-on-one support, employer outreach, and case documentation to help justice-involved participants prepare for and retain employment. Collaborate with care teams, conduct assessments, arrange employer visits, and support participant progress throughout the program.

Lead strategy and implementation of career education across sites, develop and maintain curriculum and learning technologies, manage partnerships, measure outcomes, train staff, and ensure consistent, high-quality participant career pathways and credential attainment.

Provides trauma-informed case management and reentry support for justice-involved participants after release. Facilitates individual and group life-skills sessions, monitors treatment goals, connects participants with housing, food, healthcare, and other community resources, and documents participant progress. Leads multidisciplinary care teams, collaborates with career coaches and therapists, coordinates scheduling, participates in meetings and training, and supports center events.
