Teams at Concierge Home Care

Leads a home health clinical team, supervises clinicians, reviews documentation for Medicare and regulatory compliance, monitors patient outcomes, collaborates with physicians and interdisciplinary teams, and coaches staff. The role is primarily office-based Monday through Friday, with local travel as needed. It requires an active Florida RN license, acute care and skilled home health experience, OASIS proficiency, knowledge of home health regulations, and reliable transportation.

Build hospital referral relationships, educate patients and families, coordinate hospital-to-home transitions, process referrals, complete admission documentation, and support admissions growth. The role collaborates with hospital staff, physicians, case managers, intake teams, DME providers, infusion companies, and primary care providers to promote safe discharges and reduce readmissions.

Drive home health census growth by building referral relationships, managing referral-to-admission processes, coordinating appropriate services, and collaborating with physicians, nurses, clinical teams, leadership, and referral sources. The role requires an existing territory book of business, strong sales and relationship-building skills, local travel, and reliable transportation. Compensation includes a $75,000–$90,000+ base salary and performance bonuses.
