Provide supportive counseling and care coordination to youth (ages 15-25) experiencing first-episode psychosis. Create treatment plans, connect clients to resources, assist with benefits and budgeting, deliver medications and home visits, monitor symptoms and medication response, facilitate groups, coordinate with providers, and complete client-centered documentation.

Lead and deliver IT projects—primarily an ERP implementation—and other technology initiatives (EHR, system improvements). Plan, manage budgets, schedules, risks; coordinate stakeholders, vendors, and technical teams; maintain documentation; support change management, training, and adoption; provide status updates and identify process improvements.

The role involves supporting youth with mental health needs through care planning, facilitating connections, conducting assessments, and providing therapeutic interventions under guidance.
