Provides community-based behavioral health and substance use recovery support for adults. Responsibilities include helping clients set wellness goals, build daily living and coping skills, access healthcare, housing, financial, and community resources, coordinate care and transitions, document services, and provide transportation. The role uses motivational, strengths-based support and collaborates with integrated care teams and community partners.

Transports CPRC clients safely to appointments, groups, and community services. Plans routes, maintains vehicles, completes inspections and mileage logs, communicates respectfully with clients and care teams, adapts to scheduling changes, supports staff onboarding, and follows safety, confidentiality, and agency procedures while assisting adults with serious mental illness.

Greet and check in/out patients, verify insurance and demographics, collect co-pays, answer phones and assist scheduling, enter data into EHR, maintain HIPAA confidentiality, keep front office organized, and travel between nearby clinic sites as needed.
