Teams at Community Housing Innovations, Inc.

Provides case management and supportive services for residents, including developing and updating care plans, coordinating referrals, connecting residents with public benefits and immigration services, monitoring progress, conducting room inspections, documenting interactions, and creating discharge plans. The role supports individuals and families experiencing homelessness, health or mental health challenges, and substance use issues while promoting stability, independence, and self-reliance.

Supervises case managers and oversees the development, implementation, and review of individualized service and care plans for residents. Coordinates community resources and provider partnerships, ensures compliance with program requirements, supports benefits and service access, reviews discharge planning, and intervenes in complex cases. Provides staff coaching, performance feedback, evaluations, documentation oversight, and guidance on organizational, contractual, and governmental requirements.

Coordinates educational, recreational, employment, and community programs for children and adults in supportive housing. Maintains attendance and transportation records, assists with school enrollment and employment goals, liaises with schools, DSS, and community agencies, organizes activities and fundraisers, supports grant applications, and assists case managers. Provides transportation as permitted and develops tutoring, camp, vocational, and training programs.
