Supports hospice quality and compliance initiatives through clinical data analysis, audit reporting, QAPI projects, regulatory submissions, risk investigations, survey readiness, and staff education. Reviews clinical documentation and trends, maintains tracking systems, analyzes quality indicators, supports HQRP workflows, and develops corrective action plans while ensuring compliance with Medicare, California, accreditation, HIPAA, and patient safety requirements.

Provides hospice nursing care for pediatric and other patients in their homes. Responsibilities include admissions, skilled assessments, care planning, symptom and pain management, caregiver education, direct care, documentation, interdisciplinary coordination, physician communication, crisis support, and continuity of care. The RN Case Manager also participates in quality improvement, education, rotating weekend and holiday coverage, and attends death events when required.

Provides hospice admissions and initial nursing assessments, obtains informed consent, develops care plans, orders medications and supplies, and educates patients and caregivers. Conducts home visits, triages calls, coordinates with physicians and hospice teams, documents care, supports families during end-of-life events, and provides nursing coverage for existing hospice patients. Works a full-time 10-hour shift, every other weekend, and some holidays.
