Supports hospice quality and compliance initiatives through clinical data analysis, audit reporting, QAPI projects, regulatory submissions, risk investigations, survey readiness, and staff education. Reviews clinical documentation and trends, maintains tracking systems, analyzes quality indicators, supports HQRP workflows, and develops corrective action plans while ensuring compliance with Medicare, California, accreditation, HIPAA, and patient safety requirements.

Provides developmentally appropriate grief support to bereaved children and families through school-based groups, individual sessions, counseling, curriculum development, crisis response, and community partnerships. Coordinates with school counselors and bereavement teams, documents assessments and interventions, maintains client records, collects program data, and prepares reports. The role also educates students, supports families, connects clients with resources, and participates in professional development and organizational activities.

Provides hospice nursing care for pediatric and other patients in their homes. Responsibilities include admissions, skilled assessments, care planning, symptom and pain management, caregiver education, direct care, documentation, interdisciplinary coordination, physician communication, crisis support, and continuity of care. The RN Case Manager also participates in quality improvement, education, rotating weekend and holiday coverage, and attends death events when required.
