Provides strength-based coaching, case management, counseling, and facilitation for transitional-age youth ages 14–21. Develops individualized transition plans, risk plans, and strategies supporting employment, education, housing, independence, and community participation. Coordinates services with families, schools, mental health providers, rehabilitation agencies, and community organizations. Tracks youth progress, maintains records, makes referrals, and supports continuity of care. Requires flexible evening and weekend availability.

Provides behavioral health support in an outpatient setting by completing intakes, treatment plans, reviews, behavioral rating scales, outcome measures, and clinical documentation. Supports providers and treatment teams, monitors client progress, facilitates education and counseling groups, maintains accurate charts and compliance records, and provides brief counseling while adhering to confidentiality, HIPAA, regulatory, and ethical requirements.

Coordinates referral authorizations between providers, patients, and insurance plans. Processes managed-care referrals, enters authorizations, schedules specialist and diagnostic appointments, responds to coverage inquiries, maintains referral and patient records, obtains consultation reports, supports transfers to the organization, and prepares productivity and activity reports while following HIPAA and department procedures.
