Teams at Community Health Center

Provide transportation, outreach, peer support, and supportive service coordination to connect clients with medical, behavioral health, food, housing, and community resources. Schedule and document referrals, offer appointment reminders and follow-up, maintain accurate records in agency systems, coordinate with care teams and community partners, and participate in trainings and meetings.

Provide targeted case management for justice-involved individuals pre- and post-release, coordinate care with DOC and community partners, perform outreach and enrollments, maintain EHR documentation and data reporting, conduct WRNA assessments, develop service plans, support program operations, trainings, compliance, and community engagement to reduce recidivism.

The Weitzman Education Specialist manages education interventions, supports project planning, engages stakeholders, and coordinates with faculty. Responsibilities also include data analysis and educational material development.
