Teams at Community Action Pioneer Valley, Inc.

Coordinates services for pregnant and parenting youth and young adults up to age 24. Provides case management, career and workforce development support, goal planning, resource navigation, and community referrals. Maintains participant records, data, reports, billing, and contract compliance; develops employment opportunities; supervises and trains staff; and collaborates with agencies, schools, businesses, and social service providers. Requires regular travel throughout Western Massachusetts and occasional travel outside the service area.

Coordinates a regional homelessness Coordinated Entry System, including access, assessment, prioritization, referrals, case conferencing, housing placement, provider training, technical assistance, data monitoring, compliance, and continuous improvement. Builds relationships with service providers, housing partners, government agencies, and community stakeholders while promoting equitable, trauma-informed, person-centered practices. Maintains records, supports HMIS data quality, develops work plans, identifies system barriers, and travels throughout the service region.

Provides nutrition assessment, counseling, education, food package prescriptions, referrals, documentation, and follow-up care for pregnant/postpartum individuals, infants, and young children in the WIC program. Develops educational materials and workshops, supports staff training and quality assurance, communicates with healthcare providers, maintains confidentiality, and travels regularly among regional WIC offices and outreach sites.
