Teams at Colorado Coalition for the Homeless

Provide client registration, financial screening, and enrollment for Medicaid, CHP+, Medicare Savings Programs, CICP, and Connect for Health Colorado. Verify eligibility, complete applications, maintain accurate records, support front desk/lobby coverage, conduct outreach at CCH sites, and connect clients to services and resources.

Provide peer support, advocacy, case management, and crisis intervention to residents with lived experience of homelessness, mental health, or substance use. Facilitate groups and social activities, assist with hygiene/room checks, transport residents as needed, maintain HMIS documentation, and collaborate with staff to promote recovery and self-determination.

Perform ophthalmic diagnostics and exam prep (visual acuity, visual field, OCT, pachymetry, tonometry, refraction), assist providers with procedures, triage urgent eye concerns, manage follow-up scheduling, handle optical duties (frames, lens verification), order meds/supplies via EHR, and coordinate care with medical teams.
