Provide outreach case management for individuals and families experiencing homelessness: develop and implement care plans, coordinate housing and health resources, conduct crisis intervention and safety planning, transport and accompany clients, maintain EHR documentation, track outcomes, and collaborate with community partners.

Provide trauma-informed case management and outreach for individuals experiencing or formerly experiencing homelessness. Develop and implement care and safety plans, coordinate housing and community referrals, track client outcomes, assist with benefits applications, accompany and transport clients to appointments, respond to crises, and maintain accurate EHR documentation. Work collaboratively with integrated health teams and community partners to promote long-term housing stability and supportive services.
