Teams at Clear Path for Veterans

Provides housing stabilization case management for Veterans and service members, including intake assessments, individualized housing plans, temporary financial assistance, referrals, advocacy, outreach, and follow-up. Maintains a full caseload, accurate client records, compliance documentation, and program reports. The role involves regular office and field-based work throughout the assigned Massachusetts service area, community engagement, and occasional urgent, evening, or weekend response.
