Teams at Clear Path for Veterans

Leads EOVS/HOPE-funded veteran housing programs across Massachusetts, overseeing staff, service delivery, compliance, reporting, quality assurance, partnerships, and housing outcomes. Supervises case management and peer support staff, supports complex cases, monitors HMIS documentation, develops community relationships, and implements homelessness prevention and rapid-rehousing strategies. The role is on-site and field-based, requires regular regional travel, reliable transportation, and occasional evening, weekend, holiday, or emergency support.

Provide field-based case management to veterans across Cape Cod and Suffolk County, focusing on housing stabilization, outreach, and advocacy. Conduct assessments, create housing stability plans, administer temporary financial assistance, connect clients to VA/state/local resources, maintain compliant documentation, and represent the organization at VA sites and community events.

Provide one-on-one community-based support to an assigned Veteran: accompany to appointments and activities, participate and help plan recreational/community outings, maintain activity logs, submit monthly reports, track mileage and receipts. Temporary independent contractor up to 40 hours/month with flexible weekly scheduling (day/evening).
