Collaborates with healthcare providers, social services, ICBs, hospitals, and case managers. Requires adult Registered Nurse qualification, complex care experience including tracheostomy and ventilation, a driving licence, and participation in a paid rotating on-call rota.

Provides specialist community nursing for adults with complex healthcare needs in their homes. Responsibilities include clinical assessments, person-centred care planning, client visits, care-team training and competency assessments, clinical supervision, coaching, safeguarding, quality assurance, and collaboration with families and multidisciplinary healthcare professionals. The role requires an Adult Registered Nurse with complex-care experience, a UK driving licence, and access to a vehicle.
