Provide community-based case management and support to individuals with psychiatric, substance use, medical, social, and legal needs. Conduct assessments, create and update recovery plans, coordinate services (housing, education, employment), provide transportation, facilitate groups, document outcomes, and collaborate with clinical and social service partners.

Prepare and reconcile daily transactions, month-end close, balance sheet reconciliations, bank and reserve account reconciliations, vendor payments (check/ACH), assist audits and 1099 processing, and maintain property accounting records in YARDI while supporting Controller/CFO and following internal controls and confidentiality/HIPAA guidelines.

Conduct assessments and recovery planning, connect clients to services (housing, education, employment), deliver skill-building and crisis de-escalation, document outcomes in EHRs, transport clients as needed, facilitate support groups, and collaborate with community agencies and internal teams.
