Supports meal preparation, service, delivery, and cleanup for skilled nursing residents. Ensures meals meet dietary needs and are delivered respectfully and on time. Maintains clean, safe, sanitary kitchen and dining areas, follows food safety and infection control standards, and collaborates with dietary and nursing teams to provide compassionate resident service.

Markets post-acute and long-term care services, develops hospital and community referral relationships, coordinates patient admissions and care transitions, assesses patients for admission criteria, educates families and referral sources, supports tours and educational programs, resolves referral issues, and maintains HIPAA confidentiality and visit documentation.

Lead and coordinate the facility Infection Prevention and Control Program. Ensure regulatory compliance, set IP goals, collaborate with department managers and medical directors, provide education, consultation, and act as a resource for staff.
