Provides after-hours hospice nursing visits in homes, facilities, and hospitals. Assesses patients and families, delivers symptom management and clinical interventions, coordinates interdisciplinary care, educates caregivers, completes documentation, manages admissions and discharges where required, attends patient deaths, and provides bereavement support. The role requires weekend and overnight availability, travel throughout an assigned territory, emergency response participation, and compassionate care for terminally ill patients.

Maintains hospice patients’ medical records by assembling, auditing, tracking, scanning, and finalizing documentation. Processes physician signatures and information requests, ensures HIPAA compliance, supports billing audits and quality initiatives, and assists clinical staff with record reviews. Supervises HIM volunteers and helps prepare team meeting and medical director materials. Requires strong organization, communication, confidentiality, basic medical terminology, computer skills, and the ability to work under deadlines in a healthcare office.

Analyze and abstract clinical documentation to assign accurate ICD-10-CM and CPT-4 codes, maintain coding accuracy, query physicians when needed, use encoder/CAC and EMR systems, and meet productivity and compliance standards.

Lead a team of business development liaisons to achieve admissions and growth goals. Develop and implement strategies, collaborate with clinical and access teams, manage hiring and budgets, maintain referral relationships, and ensure regulatory and company policy compliance while supporting quality improvement and community outreach.

Perform medical record coding and abstracting across service lines using ICD-10-CM and CPT-4. Review documentation, assign accurate diagnosis and procedure codes, query providers when needed, maintain a 95% accuracy rate, use encoder/CAC and EMR systems, follow AHIMA/AAPC ethical standards, and complete continuing education to maintain credentials.

Provide nursing care under RN supervision for patients and families: administer medications, perform wound/catheter care, venipuncture, IV site care, monitor and report symptoms, document care, and support patient comfort and quality of life. Follow company policies, complete competencies, and participate in emergency staffing as needed.

Provide clinical case management and palliative support to keep patients at home. Assess patient/family needs, develop and coordinate individualized plans of care with the interdisciplinary team, document in the EMR, obtain physician orders, perform home visits, supervise aides as needed, and communicate changes using SBAR. Must perform physical tasks and participate in emergency response when required.

Lead enterprise bereavement services, set standards and policies, supervise and train staff, develop programs and KPIs, ensure regulatory compliance, consult on high-risk clinical issues, and drive quality improvement and data governance across affiliates.

Provide skilled nursing assessments, coordinate individualized care plans with the interdisciplinary team, deliver palliative and supportive home-based care, document in the EMR, supervise aides, obtain physician orders, and participate in IDT meetings and quality activities to ensure patient safety and continuity of services.
