Provide direct case management to fathers: conduct assessments, create and manage family support plans, link to community and natural supports, monitor progress, coordinate services, maintain timely participant records, participate in interdisciplinary case reviews, outreach, trainings, and quality improvement while ensuring confidentiality.

Supervise and support case management staff for the FRANC fatherhood program, maintain data systems and reporting, implement quality assurance and continuous improvement, provide direct case management as needed, build community partnerships, and participate in outreach and interdisciplinary case reviews to improve outcomes for fathers.

Provide lactation consultations (in-person, phone, video), triage participants, schedule and assign cases, document charts, follow up and refer clients, deliver and maintain breast pumps, cover staff at program locations, and participate in trainings and quality assurance under program supervision.
