Teams at Central Jersey Family Health Consortium, Inc.

Coordinates client-centered care for maternal-child health families, connecting them with community resources and providing information, support, advocacy, follow-up, and care coordination. Manages referrals from DCPP, assesses family risks and protective factors, collaborates with caseworkers and service providers, supports families with complex needs, maintains a limited caseload, and participates in quality improvement and evaluation activities. Local and statewide travel using a personal vehicle is required.

Coordinates Family Connects home-visiting program enrollment, nurse scheduling, client database management, appointment confirmations, recruitment, and post-visit follow-up. Builds relationships with hospitals and birth centers, connects families with referrals, assesses satisfaction, and provides re-referrals when needed. Serves as the primary public contact, attends program meetings and trainings, and supports families including those experiencing infant loss. Requires culturally sensitive communication with diverse and underserved populations, bilingual English-Spanish fluency, accurate data entry, and local and statewide travel.

Coordinates enrollment, scheduling, and follow-up for families in a maternal-child health home visiting program. Responsibilities include recruiting and engaging families, managing nurse assignments and client databases, scheduling and confirming visits, conducting post-visit calls, tracking referrals, answering questions, and rescheduling appointments. The role also builds relationships with hospital birth centers, participates in team meetings and trainings, and serves diverse and underserved populations.
