Prepare and serve meals per patient dietary specifications, pass trays, wash dishes, clean and stock kitchen/dining areas, follow directions, and work weekends as part of a team in a skilled nursing facility.

Provide direct patient care in a skilled nursing facility: assist with ADLs, answer call lights, shower aide duties, charting, collaborate with care team, and follow state/federal regulations to support rehabilitation and discharge planning.

Coordinate weekend admissions, transfers, and discharges for a skilled nursing facility; document preadmission paperwork in the EMR; perform insurance authorization, scanning/faxing/emailing, and maintain HIPAA confidentiality.
