Provide patient placement referrals, perform assessments, develop and implement care plans, maintain patient records, support audits, and complete other assigned case management duties to coordinate patient care.

Prepare and serve nutritious, appealing meals; set up and maintain clean dining areas; assist in food preparation, storage, and labeling; follow proper food handling, health, and safety procedures; interact courteously with patients and staff in a fast-paced environment.

Provide direct occupational therapy services to sub-acute inpatient residents under the supervision of a Licensed OT. Treat patients with orthopedic, neurological, cardiopulmonary, and complex medical conditions, document care, and collaborate with an interdisciplinary team to support rehabilitation goals.
